I use a heavy scull cap of most of my winter riding. When it gets way down I will add a hood.
For some reason I have never found a need to cover my face - but that is just me - I have gone as much as 4 layers on my legs.

REI's thick ear band.  Not a bicycle item, but toasty warm.

If it gets colder than that, balaclava (thin) added beneath the ear band.

As it gets even colder I begin adding windproof layers of hood/face guard/helmet cover.  The face guards you can get as ski items.

I have a winter helmet that is a size too large to accomodate all the thickness.

Cheap walmart skull cap, if more is needed (30 degrees and wet) I add a cheap polypro balaclava.  -- Nelson.

I have a Turtle Fur headband that is actually for skiing. It covers my ears well but leaves the top of my head open so I don't overheat.

Hay I got one of those for Christmas and my wife pick them up and they were gone.  She likes them because it wont mess up her hair. For some reason I don't have that kind of problem.  (Hair that is)

I use a Polar Fleece 'clava and a full face helmet. when things get really cold I have a motorcycle helmet with a face shield and a baffle to keep my breath from fogging the shield. I have ridden down well below freezing with these setups.I got the idea from a guy in Canada that routinely rides in temperatures more than 20 below 0 C.BTW he's insane.

Most of the time what Paul said.  This morning it was 31 degrees ("feels like 24") and I just wore a fleece balaclava.  One layer.  Fleece is the bomb!

I just use a cycling cap under the helmet, you can get them fairly cheap at RBM
